Louisiana-Double Fatality Crash In Rapides Parish

On Monday, September 8, 2008, at 10:32 pm, Louisiana State Police responded to a fatal crash involving two vehicles.  The crash occurred on LA Hwy 488, milepost 15.5, in Rapides Parish.

The crash involved a 1997 Ford pickup, driven by Mr. Jonathan W. Afeman (W/M 25 yrs) of Hineston, LA and a 2004 Dodge pickup, driven by Ms. Theresa Coutee (W/F 28 yrs) of Alexandria, LA.  The Ford pickup was east bound on LA Hwy 488 when Mr. Afeman lost control and exited the right side of the roadway.  After exiting the roadway, the Ford collided with a tree prior to coming to rest.  The Dodge was also east bound on LA Hwy 488 (behind the Ford) when Ms. Coutee was forced to take evasive action in an effort to avoid the Ford.  Ms. Coutee swerved to the right, which caused her to exit the right side of the roadway.  After exiting the roadway, the Dodge collided with several trees prior to coming to rest.  

Mr. Afeman was transported to Cabrini Hospital where he was pronounced dead.  He was wearing a seatbelt.

Ms. Coutee was pronounced dead at the scene.  She was wearing a seatbelt.

Rapides Parish Coroner’s Office will conduct an investigation to determine the exact cause of death.

Routine toxicology tests are pending.

The crash remains under investigation.

Troop E Troopers have investigated 43 fatal crashes for the year of 2008, resulting in 48 fatalities.  Six of the 43 fatal crashes have involved pedestrians.  Five fatal crashes have involved an ATV and two have involved a motorcycle.    Twenty-four of the 30 fatal crashes (passenger vehicles only) have involved unrestrained motorists/occupants.  Eight fatal victims were restrained.
